As an aside, many people call NIV “BiPAP”. For the sake of communication I know it’s embedded in the lexicon to just call it “BiPAP”. NIV (non-invasive ventilation) encompasses bi-level other modes as well. BiPAP is the proprietary name for bi-level on the Philips machines. #FOAMed

One of the first lessons in #internalmedicineresidency and #emergencymedicineresidency is how to treat #DKA. We worry about the serum bicarb being low and immediately get itchy to just give some to make the number pretty and fix the pH/acidosis. Side note. If you’re curious about the pH, get a VBG instead of an ABG; less discomfort for the patient. Using CRP to help one decide if Dexamethasone is appropriate or not in COVID. FOAMed.

Really quick because I need to go to bed and get some rest before I head back into COVID world tomorrow. This is an observational study out of NYC where they looked at the response of patients to corticosteroids depending on whether on their CRP. This is not a prospective trial and therefore should not be used to guide any management at this point, but it still is quite interesting. Take a look for yourself. It’s FOAMed!

I like these folks in the BMJ. “The guideline panel makes a weak recommendation for the use of Remdesivir in severe covid-19.” I agree. Intensivist Deficiency: FOAMed

This morning, I woke up to an email from “Today’s Hospitalist” with a headline regarding physician workforce. It reads “Intensivists are in short supply in more than a dozen states”. I don’t know about many of my colleagues here, but I get hit with calls from head-hunters all the time for moonlighting/locums opportunities. My phone has been buzzing quite a bit. George Washington University put out a report (CLICK HERE FOR REPORT/PDF) of how critical (pun intended) things are in certain states.

The other cool thing about this PDF is that they have a link to a “Workforce Deficit Estimator” (CLICK HERE FOR THAT LINK). I wouldn’t say that this looks accurate for nurses and other staff, but it appears to be accurate for Intensivists. Are you interested in becoming an Intensivist? We need your help. Being a Zentensivist

Being on social media has blessed me with the opportunity to be exposed to the works of great people and work with some fascinating brains. I have been on the FOAMed train since I was an intern and have long admired people who I am routinely in contact with today. Recently, I have joined on as an editor on criticalcarenow.com with Haney Mallemat (@criticalcarenow). Amongst the other individuals contributing with content to this website, I have run into the work of Matt Siuba (@msiuba, also an editor on criticalcarenow.com. Being on twitter, despite far less “cool” there, I had run into this concept of “Zentensivist” which was floating around the ether there. Matt is the lead author of this paper. Josh Farkas (@pulmcrit) of pulmcrit.org also contributed to this paper.

This concept of Zentensivist resonated with me because practicing Critical Care, although as chaotic as can be, needs to be performed in the highest state of flow possible. This applies to being the physician, APP, RN, RT, CNA, and other key members of the team. Chaos goes nowhere. Yelling goes nowhere. In my opinion, it reflects a lack of leadership, confidence, emotional intelligence, and self awareness.

There is a great flowchart in this article which demonstrates the philosophy I innately attempt to go by in a nice visual form. You can download the paper for free to see it for yourself. If you’re a student, trainee (intern, resident, fellow), or anyone taking care of the critically ill, this is what you should aspire to achieve. This also applies to the rest of the team taking care of these patients. FOAMed

It all starts with being a calming presence. Yelling, accusing others, talking smack helps no one. Remember, the #1 priority is the patient, not your ego. The other zentensivist principles will not show up on the first day. They come with years of practice, experience, and self-improvement. Your days of ordering every test under the sun are excusable during training, not as a seasoned veteran. We all need to get better at this. Hat tip to the authors.

Tocilizumab for COVID-19 Patients on Mechanical Ventilation FOAMed

Thus far, the only thing that has been shown to decrease mortality in patients with COVID-19 is dexamethasone. Now, there appears to be a second agent. Tocilizumab blocks IL-6. This was an observational trial looking at 154 patients. The two groups were NOT evenly matched. That being said, the patients did have improved survival, although they did have a greater amount of secondary infections. Pneumonia was the most common of these. We are currently using this therapy for our patients as well as enrolling many into studies utilizing similar therapeutic medications. Enjoy this piece of FOAMed!

ARDS and COVID-19: Who cares about making the diagnosis?

We love to write “mild, moderate, or severe ARDS” in the chart based on the P/F ratio of the patient and the Berlin Criteria. It makes us feel smart. Based on the P/F ratio, historically, we perform certain interventions like employing paralytics, proning or intubating the patient. In this paper written by Martin Tobin, he questions this whole concept in the face of ARDS in COVID.

For those of you who do not know who Martin Tobin is, well, he’s the person who literally wrote the book on mechanical ventilation. It’s titled “Principles And Practice of Mechanical Ventilation, Third Edition“. If you are interested in a more to the point book on ventilators, I recommend “The Ventilator Book” by Will Owens. Summary on IV Fluids: FOAMed

The paper linked below is from the International Fluid Academy. I did not even know such institutions existed! I want to be part of that team! Some big names on this paper that I would love to be in the same room with. Nonetheless, the fanboy in me needs to digress. I plan on breaking down this paper more in detail but I simply do not have the time this week. I am sure many of you will love it as you’re as fascinated by fluids as much as I am. If you all call for a breakdown loud enough, I’ll get to it. This was published two days ago. Some great FOAMed here!

Stop ordering daily chest x-rays! This post came about after @memenurseofficial posted something about radiology messing up the sheets in the ICU at 6am that was quite comical. It also spurred some complaints about daily CXR. I commented about it being somewhat wasteful to mindlessly check daily CXR’s and here is the link to the article:
